How do I schedule my first appointment?
Do you have parking on site?
What should I bring to my first appointment?
When arriving in person, please bring:
– Your insurance card (we will make a copy)
– A prescription or referral (if needed) from your referring physician
– For Motor Vehicle Accidents and Worker’s Compensation, please have a claim number, insurance adjuster, and a lawyer’s contact information ready
– Please arrive 10 minutes early to complete any remaining forms
– Please wear clothing that is comfortable and allows access to the injured areas
