What should I bring to my first appointment?
Before your initial visit to our clinic, please fill out our patient intake forms online. This will help streamline your check-in process and ensure we have the necessary information to provide you with the best care possible.
When you arrive in person, please bring the following:
- Insurance card: We’ll make a copy to verify your coverage.
- Prescription or referral (if required): Only needed if your referring physician provided one.
- Motor Vehicle Accident or Worker’s Compensation details: Make sure to have your claim number, insurance adjuster’s name, and lawyer’s contact information ready.
Please arrive 10 minutes early to complete any remaining forms. This will help us start your session on time and make the most of your visit. We also recommend wearing comfortable clothing that allows easy access to any injured or affected areas. This helps our physical therapists conduct a thorough examination and provide effective treatment.
Does insurance cover my treatment?
